The dedicated team of skilled medical professionals places an emphasis on preventive care and fitness for optimum health. We are also prepared to support residents as needs change.

The Health Services team at Plymouth Harbor is equipped to serve our residents should they face sudden obstacles. Our residents have priority access to home health services, care management, assisted living and memory care facilities, as well as rehabilitation and a skilled nursing facility within the community. There is no need to worry or dwell on the “what ifs” of retirement when you live at Plymouth Harbor.

 

Assisted Living and Memory Care

Plymouth Harbor offers assisted living in the Seaside and Starr Memory Care Residences. Licensed nurses and care partners provide the support needed to maintain a high level of independence with all-inclusive assistance with activities of daily living. Our robust services include meals, housekeeping, laundry, medication administration, programs, call system, and coordination of medical care.

In Seaside, we provide our residents with the support and assistance needed to remain independent and engaged in community life as long as possible. Offering spectacular views and several cozy spots to enjoy them. The Seaside is an intimate home of only 30 apartments, ensuring superb individual attention. At the same time, residents are welcome to enjoy the many amenities and programs of Plymouth Harbor or the fantastic offerings around Sarasota.

Two secure neighborhoods comprise Starr Memory Care, marked by luxurious surroundings, spacious suites, and state-of-the-art design for living with dementia. A high staffing level of nurses, care partners, and support staff provides 24-hour care and compassion for our residents. Our Education and Outreach Coach ensures continuous education and coaching for our team members and families on interacting positively with those living with dementia.

Home Care and Community-Based Services

Plymouth Harbor offers home health services in the privacy and comfort of the resident’s home, coordinated and supervised by a registered nurse. Among the services offered through our home health services are planning for care, which identifies the resident’s goals and the potential obstacles he or she could face. Recovering from injury, surgery, illness, or disease are also reasons why one might be seeking home health services. Self-management begins with understanding and identifying symptoms and managing medications.

Plymouth Harbor’s caring approach to the delivery of home health services focuses on the individual needs of our residents and those closest to them.

If you are a resident of Plymouth Harbor, or are seeking assistance for your loved one who is a resident, our home health services are available to you. Among the specific services provided are:

  • Complimentary assessment by a registered nurse
  • Skilled nursing care (RN or LPN)
  • Medication set up
  • Medication administering
  • Assistance with personal care, light housekeeping, light meal prep, or laundry
  • Accompaniment to physician appointments
  • Private duty assistance for companionship, respite care, arranging appointments, or organizing mail, assistance with groceries, errands, or assistance with pet needs
  • Clinic Services– respond to health emergencies across campus, flu shots, COVID testing, lab draws, and blood pressure checks.

Home Care Management

Plymouth Harbor offers Care Management services as an option to provide additional support and advocacy as a client navigates through the continuum of care.This team of medical professionals monitor medical needs and coordinate care on an ongoing basis. Upon selection of this service a concierge registered nurse care manager is assigned to oversee client care in all settings. The nurse is on-call for emergency room visits and collaborates with all health providers to ensure continuity of care.

The care manager provides education and guidance related to medical conditions and treatment options. They also identify and coordinate in-house and community resources to promote overall well-being. They act as a liaison managing medical needs appointments test procedures and assess with insurance claim appeals and activation of long-term care policies. Care management is an ideal option for clients whose family members are not local or readily available to assist. Care managers provide frequent updates to responsible parties and health care providers on behalf of the clients served.

Smith Care Center

Plymouth Harbor has a proud tradition of providing skilled nursing and an excellent continuum of care to its residents. At Smith Care Center, we empower patients and residents to take charge of their health care journey. Under the order of a licensed physician, we offer comprehensive skilled nursing services which are personalized for the unique needs of the individual. Additional support is provided through our social, recreational, therapeutic, and dining services.

The Smith Care Center offers comprehensive skilled nursing services, which are personalized for the unique needs of the individual. Under physician’s orders, licensed nurses along with compassionate, dedicated staff treat residents and patients with dignity and respect. Residents have an option of selecting a private or semi-private room with space for decorative personal items that offer a sense of familiarity. Rooms offer serene bay, garden, or pond views. Residents may choose between receiving medical oversight from the center’s Medical Director or from their established physician in the community. The Smith Care Center works collaboratively with Sarasota Memorial Hospital and Doctors Hospital, should the need arise for hospitalization.

Medical assistance is just one of our pillars of support offered in the Smith Care Center. We also provide a full selection of social, culinary, rehabilitation and recreational programming. Residents are encouraged to participate in outings of their choosing and to maintain an interest in their favorite pastimes and hobbies.

Short Term Rehabilitative Services

Also housed within our Smith Care Center at Plymouth Harbor is a short-term rehabilitation program. The program incorporates a multi-disciplinary approach to individual needs, all focused on a goal of returning you to your home. Our physical, occupational, and speech therapists work together to restore quality and purpose of daily living for those with a serious illness or injury, or recovery from elective surgery.

The Smith Care Center short term rehab program welcomes residents from within Plymouth Harbor, as well as community members in Sarasota and the surrounding area who are recovering from an injury or surgery and require rehab before returning home.

As Smith Care Center is Medicare approved, Medicare provides reimbursement for up to 100 days, depending on:

  • The patient’s medical condition;
  • Physician’s certification that the patient requires rehabilitative care;
  • Whether the patient has spent three consecutive days in an acute care hospital, not counting the day of discharge.
